Welcome! We’re excited to begin this journey with you. The steps below will guide you through setting up your secure patient portal for the first time.
Look for an email with the subject line:
“Synapse Solutions welcomes you to the Patient Portal”
📧 This email will be sent from DoNotReply@alert.insynchcs.net.
The email will include a secure link to set up your account. This link will expire after 72 hours, so be sure to click it as soon as possible.
📱 If you're using a smartphone: It's best to open the link into a separate mobile browser instead of opening it inside your email app’s built-in browser. Later in the setup process, you’ll be prompted to retrieve a security code from your email, and switching between apps is much easier this way.
🔍 Note: The portal works best in Google Chrome, but can be accessed using most browsers. If you're prompted to open the link in Chrome, you can simply close the prompt and continue in your current browser.
Since it’s your first time accessing the portal, you’ll be asked to create a secure password. Follow the on-screen prompts.
💡 Tip: Write down your password and keep it somewhere safe.
Once your password is set, use your username and password to log in at any time.
🎉 Voila! You did it!
After logging in, click “Forms/Documents.” Complete any assigned forms. Be sure to also fill out the History Form found under the “History” tab.
✍️ You can electronically sign and submit everything directly within the portal.
Once all forms are completed and submitted, give us a call at 608-580-0031. A member of our team will review your information and help you schedule your first appointment.
