If we receive a referral from another provider on your behalf (such as a physician, therapist, or another provider), a member of our team will reach out to you directly to begin the intake process.
Once you're ready to move forward with services, we’ll create your portal account.
You’ll then receive an email invitation from InSync (Qualifacts) with instructions to set up your secure login and password.
