Look for an email with the subject line: "Synapse Solutions welcomes you to the Patient Portal". The email will be sent from DoNotReply@alert.insynchcs.net.

Step 2: Click the link in the email.

The email will contain a link to access the patient portal. Click on the link to open the login page. The link will expire after 72 hours.

Note: The portal is optimized for Google Chrome, but you can still access it using other browsers. If a prompt appears asking you to open the link in Chrome, you can simply close it (click "X") and proceed with your current browser.

This is your first time accessing the portal, so you'll need to create a password. Follow the prompts on the login page to set up your password. Be sure to write this down somewhere you can find it later!

Once you've created your password, use your login credentials (username and password) to access the patient portal.

Once logged in, click on "Forms/Documents." Locate and complete any forms assigned to you. Don't forget to complete the history form under the "History" tab. You can electronically sign and submit the forms within the portal.

Once you've completed, signed, and submitted all the required forms, call Synapse Solutions' office. A staff member will review your documents and assist you with scheduling an appointment.
